This consent supports timely and equitable access to care that promotes student safety, wellness, and readiness to learn. Consent is valid for the entire school year unless you choose to revoke it in writing. Students without consent will default to declined consent.

Schedule pickup begins Tuesday, July 30th. To pick up your schedule, you must have completed online registration AND uploaded your proof of residency. The ONLY acceptable proof of residency documents are: current gas, water, or electricity bill OR Mortgage or lease agreement in the name of the parent/guardian. All documents MUST be dated within the last 30 days.
